What You'll Do:

As a key member of our interdisciplinary team, you will provide a full range of physical therapy services to a diverse caseload of home health clients. Your expertise will be crucial in helping individuals with neurological, musculoskeletal, orthopedic, and cardiopulmonary conditions regain their independence and improve their quality of life.

Key Responsibilities:

• Conduct comprehensive initial assessments and develop individualized treatment plans.

• Deliver evidence-based physical therapy interventions within the client's home environment.

• Educate clients and their families on therapeutic exercises, adaptive equipment, and strategies for safe and independent living.

• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and effective care.

• Maintain timely and accurate electronic documentation of all visits and patient progress.

• Participate in discharge planning to ensure a smooth transition for your clients.

What We're Looking For:

We are seeking a dependable, energetic, and empathetic individual with strong clinical and interpersonal skills.

Position Qualifications:

• Valid and unrestricted Physical Therapist license in the State of Wisconsin.

• A strong sense of initiative and the ability to work independently.

• Excellent organizational and time management skills to effectively manage a caseload and travel route.

• Proficient computer skills for electronic charting and communication.

• A valid driver's license, reliable automobile, and current auto insurance.

• Must pass a pre-employment physical, drug screen, and background check.

• Current TB skin test (or willingness to obtain one upon hire).

• A positive and professional demeanor with a commitment to patient-centered care.

• Previous experience (1-3 years) in a home health setting is a plus, but new graduates with a passion for home care are encouraged to apply.

Physical Requirements:

This role requires the ability to travel to and navigate within client homes. The position involves regular sitting, talking, and hearing, as well as occasional standing, walking, stooping, and lifting up to 50 pounds to assist with patient mobility and equipment.
