Professional Home Care Services, Inc. (PHCS) is a licensed, mid-sized home health agency with over 30 years of experience serving the Milwaukee community. We’re currently seeking an experienced and dependable part-time Personal Care Worker (PCW) to join our family-oriented team and provide excellent care a client in the Wauwatosa.

SCHEDULE: Saturday and Sunday (6.5hr/day)

As a Federally and State Licensed agency, PHCS provides a higher level of skilled service. We are seeking a professional PCW who is looking for a rewarding career, not just a “sitter” position. This role is ideal for a caregiver whose skills include transfers, catheter care, bowel programs, and more.

Primary Responsibilities

Under the supervision of an RN Case Manager, you will provide hands-on care and assistance with daily living activities based on a written Care Plan.

• Provide hands-on personal care, including bathing, personal hygiene, grooming, toileting, and dressing.
• Assist clients with transfers, ambulation, and range of motion exercises, using proper body mechanics and equipment (e.g., Hoyer lift).
• Perform specialized skilled tasks as trained, such as catheter care (changing drainage bags, applying leg bags), and application of non-sterile dressings.
• Plan and prepare nutritious meals and snacks according to the client’s therapeutic diet.
• Perform light housekeeping tasks to maintain a clean, safe, and healthy home environment.
• Report all observations and any changes in a client’s condition immediately to the RN Case Manager.
• Accurately document all care and observations from your visit using our Electronic Visit Verification (EVV) system.

Job Requirements

• Minimum 1 year of experience as a Personal Care Worker (PCW) is required.
• Previous experience in a home health care setting.
• Experienced, able, and will to do the Bowel Routine (this is required)
• Experience caring for clients with MS
• A valid driver’s license and a reliable, insured vehicle to travel between client homes in Metro Milwaukee.
• Must have a positive attitude, be a team player, and be able to accept and take direction.
• Must be reliable, on-time, and able to keep a consistent schedule.
• Able to pass a pre-employment physical and drug screen.

Preferred Qualifications

• Experience caring for clients with dementia, ALS, MS, or quadriplegia.
• Solid PCW skills including transfers, catheter care, bowel program, range of motion, etc.
