About the Opportunity

Professional Home Care Services, Inc. (PHCS) is currently seeking an experienced and dependable Personal Care Worker (PCW) to join our family-oriented team. As a licensed, mid-sized home health agency with over 35 years of experience serving the Milwaukee community, we pride ourselves on providing a higher level of skilled service.

We are looking for a professional caregiver who wants a rewarding career, not just a “sitter” position. This role is ideal for a skilled PCW, CNA, or Nursing Student looking for consistent morning hours with a local, established agency.

The Schedule

• Core Shift: Monday through Friday, 8:00 AM to 11:15 AM.

• Flexibility: This is a perfect set schedule for parents dropping kids at school or students with afternoon classes.

• Growth: There is a strong possibility to pick up additional clients and hours after 11:15 AM if desired.

Primary Responsibilities

Under the supervision of an RN Case Manager, you will provide hands-on care and assistance with Activities of Daily Living (ADLs) based on a written Care Plan.

• Skilled Personal Care: Provide hands-on assistance with bathing, personal hygiene, grooming, toileting, and dressing.

• Mobility Assistance: Assist clients with transfers, ambulation, and range of motion exercises using proper body mechanics and equipment (e.g., Hoyer lift, gait belt).

• Specialized Care: Perform tasks such as catheter care (changing drainage bags, applying leg bags), bowel programs, and application of non-sterile dressings.

• Home Management: Plan and prepare nutritious meals according to therapeutic diets and perform light housekeeping to maintain a safe home environment.

• Communication: Report changes in client condition immediately to the RN Case Manager and accurately document visits using our Electronic Visit Verification (EVV) system.

Job Requirements

• Experience: Minimum 1 year of experience as a Personal Care Worker (PCW) or Caregiver in a home health care setting is required.

• Transportation: Must possess a valid driver’s license and a reliable, insured vehicle to travel between client homes in Metro Milwaukee.

• Reliability: Must be on time, reliable, and able to maintain a consistent Monday–Friday schedule.

• Health Requirements: Ability to pass a pre-employment physical and drug screen.

• Attitude: Must have a positive attitude, be a team player, and be willing to follow RN direction.

Preferred Qualifications

• Experience caring for clients with high-need conditions such as dementia, ALS, MS, or quadriplegia.

• Strong competency in transfers, catheter care, and bowel programs.
